This post provides information on process to include name into birth certificate for child born in Bangalore BBMP Limits
Case in Point:
My niece who was born in Malleshwaram Bangalore in 1994 had got a birth certificate without her name mentioned in it. It was all fine, she was able to complete her graduation with this certificate, but when she wanted to apply for passport, it was mandatory to have birth certificate which mentions her name.
Process:
Documents Needed:
Photo copy (xerox) of following:
BBMP Office:
Visit BBMP office in Majestic, its building next to Upparpet Police Station.

Timing: Monday to Saturday, opens at 10:00am, suggest go earlier, you can beat the crowd.
There is an application form, which has text both in Kannada and English, so should be easy for non kannadigas to fill it up, there is field to mention child name, mentioned it the way you want the certificate, fill other details in the form and submit it along with the documents mentioned above. Only parent of the child is recommend to sign this application form.
There are two computerized counter which accepts these application form, I submitted it, the person at the counter checked my documents and mentioned father name would be only first name, last name was not mentioned in the old birth certificate, if you want last name to be included then you need to get a letter mentioning the change and signed by both the parents. In my case both parents were not present, so I was OK with only first name. What was important was the child name.
I received an acknowledgement receipt, mentioning to visit on 6/04 - 3PM to collect birth certificate. I paid Rs. 120/- for 3 copies.
The way entire process was done and the person knowledge at the counter and the computerisation process gave me the confidence that it should be done with in specified time.
Collection Process:
On the specified date and time, I visited BBMP office, ground floor there is a dispatch counter, where I presented my receipt, person checked a bundle and mentioned 1st floor, went to 1st floor computer section, there were two ladies, both ladies checked bundles with them couldn't find my certificate, one of them checked on her system and mentioned it was processed yesterday, spoke to another girl in the room got one more bundle, found my birth certificate handed it over to me. I thanked them and walked back..
All the time, I was praying that they don't ask me to come back tomorrow, that's what happens in most of the govt. departments.
But, Wow...I was able to get my required documents in just two visits, once for submitting the application and 2nd for collecting.
Most surprising fact was no one asked or expected money for this service.

I followed your post. However there is a small twist. My son was born in Vijayanagar Global hospital. First I contacted Upparpet and then Mayo hall BBMP citizen centers. The name inclusion can be done only in the zone where the hospital is attached. Actually there is no clarity...They asked me to go from Mayo Hall to jaya nagar and then to BBMP office in malleshwaram. I really dont get the logic on what basis vijayanagar global hospital is attached to a BBMP office opposite to mantri square. Only health dept can answer which zone every hospital belongs
At last at BBMP malleshwaram.There they accepted Rs 50, apllication form and id proofs. Time for processing is 15 days.

In such cases, best would be to visit that particular hospital and check currently which BBMP office has their records, that should reduce going around.
Parents are physically required to be present to get this job done.? As my son born in Bangalore and then we moved outside India. Now for visa purpose i need to include the name on his birth certificate and I am not able to find a way as if that can be down through one of my friend who is in there? please let me know if you have any details around this?
I just wanted to share my experience. I had visited the bbmp office at mahadevapura for name inclusion. My initial thought was I may need to visit at least twice. To my surprise the entire process was done in 5 mins. I met a gentleman at the counter who asked me the filled form and birth certificate without name and then gave me a print out certificate with name and sealed. Wow amazed and happy.
Hi Anupam, Appl. Form mentions it has to be signed by parents only, what I can suggest is download the application form from BBMP site, sign and send it across to your friend in Bangalore, who can submit it.

I born in 1989 but my first name in the birth certificate is different. When i checked with BBMP office at Upparpet, they advised to get court order and asked to consult lawyer. Lawyer charges approx of Rs 3000, i din't understand this. Its so annoying.. Plz suggest

By going through some documents on the net, what I see is, if you provide your id proof along with an application form, that should be sufficient to get it corrected, please verify on this.

I got a blank birth certificate from the hospital. When I asked the hospital, they asked me to go to BBMP office in Shanthi Nagar but when I went there, they said they only process property tax and voter IDs!
Not sure where to go now, Majestic, Mayo Hall, Malleswaram, jayanagar, Ulsoor or Mahadevpura??
Similar experience. The process was pretty fast and got the certificates same moment. Our hospital is Cloudnine Bellandur that gave blank certificate. They charge Rs. 600 for name update work.
So I went to Whitefield BBMP office opposite Phoenix Marketcity. They gave small form to fill. As we were within an year, no documents were required. The person at the counter issued 3 certificates and charged Rs. 210, without any payment receipt. Didn't question the charges as all we're paying, but apart from the payment, which mostly looks like they are pocketing, the process was hassle-free.

Mentioned date above since rules and details keep changing. What is described below is valid today :).
Had to get name inclusion done in my niece's Birth Certificate - she's a 11 year old and currently abroad with her parents. Went first to Utility Building, security person checked DOB and told that:
1) BC issuance where DOB falls before 2010 will be handled in the BBMP office near Upparpet Police Station (location provided by Suresh in this post, apparently this is the head quarters and all old cases will be handled here)
2) Post 2010 (I think 2010 included, not sure) will be handled in Utility Building
Went to Upparpet BBMP office and filled 2 forms (Issuance of BC, Name inclusion), submitted them along with Copy of original BC (without name), Copy of School TC (didn't submit or show any originals). Paid Rs.140/- for 5 copies. Have been asked to come on 20 Nov, 2017 (a 10-day lead time). Process was easy, let's wait and see if they deliver as promised and on time.

I followed the steps and was able to submit the documents in Majestic. However, when I came back I saw in the receipt that Name field had my name not my Child's Name. Received from also had my Name. Is this how you also got the receipt?

I checked my receipt, as you have mentioned only father name is printed on the receipt, no issues, if you have filled in the form correctly, that's how the certificate would be printed.
Things are improving in BBMP front in this department, so you should get the certificate soon, good luck.

No, it can't be done online.
Just to share a recent incident which happened in Bangalore, my son wanted bus pass for which I downloaded application form BMTC website, filled up and he went to bus depot to submit, it was rejected stating "This is not Modi government to get form downloaded from internet" When an application form downloaded isn't accepted, please do not expect to have such critical task to be done online.

Hi Radhika, if child was born before 2010 then go to Upparpet BBPM office.
If born after 2010 then go to Utility Building MG road BBMP office, please read Hopeful Citizen comment above for more details.

I'm able to request for the Name addition in Birth Certificate. Few information that will be useful for others.
1) To check for the BBMP Office to visit, Please check with your Hospital. (For Motherhood HRBR Layout, it is BBMP Frazer Town behind Government Hospital over there)
2) If the child is below 6 years of age, Only document required is "Photocopy of Birth Certificate without Name" along with the two forms (which we can get from the BBMP Office) to be filled by us.
3) Submit the two forms filled along with the "Photocopy of Birth Certificate without Name" in the counter where you have collected the form. They will ask the money according to the number of copies that you have requested.
4) They will give you one chit or receipt mentioning when you have to collect the birth certificate.

As per info, I could gather, you should be visiting Utiltiy Building office in MG Road for all births post 2010.
